Service Coordination and Navigation (SCaN) youth intervention program is an intensive, community-based model that connects youth ages 14-24 at the highest risk of violence involvement to services in support of goals that increase their stability, self-efficacy, and assist in their transition to adulthood.

The Service Coordination and Navigation (SCaN) program is a strengths-based, youth-driven program model that connects youth ages 14-24 at the highest risk of violence involvement to services that meet their individual needs and increase their stability, self-efficacy, and assist in their transition to adulthood.

Participating youth are paired with a full-time “Navigator” who helps them identify and accomplish individualized goals. Navigators connect youth to programs, services, and benefits necessary to build a stable pathway to success. SCaN partner agencies are deeply embedded within the community to provide services in the spaces where participants live and develop strong communities.

CRP funding supports approximately 22% of the Service Coordination and Navigation program. Data presented on this page is specific to the CRP-funded portion of the program.
